Kindly read the Instructions carefully before filling the Application Form.

1. The Form can be filled through a Mobile, Tablet, Desktop PC or a Laptop.  Kindly ensure these devices are logged in to a GMAIL Account, as the form will be filled through Google Form.

2. Filling of more than one form would lead to rejection of all your filled Forms.

3. Read carefully all questions asked in the Form and then enter the details accordingly.

4.  Kindly fill all details correctly.  If its found later that the details filled in the Application Form are wrong or should there be any discrepancy, the Form would be rejected.

5. The Form has four sections:-

• • Child's Information

  • Father's Information

  • Mother's Information

  • Siblings Information

After filling all the four sections, Click on SUBMIT and wait for a Confirmation Message.

A Email will be sent to you from St. John Marie Vianney KG Classes with a copy of the Form.  In case if you do not receive the mail in your INBOX, kindly check the SPAM.  In case if the mail is received in the SPAM, move it to the INBOX and then download the Application Form.  It would take 15-30 minutes to receive the mail.  Kindly take a Printout once you have received the Application Form

6. Once the Form is filled, no changes will be accepted.  

7. All information related to your child's admission will be intimated to you through Email / SMS / WhatsApp only.
